About MassHealth LTSS Public Assessor Option

The public assessor option (PAO) can complete assessments for members seeking any of the services listed below. An assessment is a review where a nurse will talk with you about your needs to help find out if you qualify for a service.

1. Adult Foster Care

Live in your own home or in a qualified caregiver’s home with support from that caregiver.

Adult Foster Care may help MassHealth members who

• Have a medical or mental health condition
• Are age 16 or older
• Need hands-on help, reminders, or supervision with daily activities, such as bathing, dressing, eating, moving around, or using the bathroom
• Can live safely in a home setting with caregiver support
  • The caregiver may be a family member or a certified foster caregiver but cannot be the member’s spouse or legal guardian.

2. Adult Day Health (ADH)

Go to a daytime program to get medical care and help with daily activities, while still living at home.

Adult Day Health may help MassHealth members who:

• Are age 18 or older
• Need nursing support or help watching for health changes, such as changes in blood pressure, medication needs, or symptoms
• Need help with daily activities, such as bathing, eating, walking, or using the bathroom
• Can attend a daytime program in a safe group setting
• Return home after the program day

3.  Group Adult Foster Care (GAFC)

Get personal care support from health care staff where you live. This may be in an assisted living facility or other supportive housing.

Group Adult Foster Care may help MassHealth members who:

• Need help, reminders, or supervision with daily activities, such as bathing, dressing, eating, moving around, or using the bathroom
  
• Live in an approved housing setting where Group Adult Foster Care services are available
  
• Can live safely with support
  
• Need services to help them stay in the community services.

4. Personal Care Attendant (PCA) Services

Get help with daily Activities at home or in the community. 

Personal Care Attendant services may help MassHealth members who:

• Have a long-term disability
  
• Need physical help with daily activities, such as bathing, dressing, eating, moving around, or using the bathroom
  
• Can manage their own Personal Care Attendant services, or have someone help manage them
  
• With this service, the member hires and manages their own Personal Care Attendant. If needed, another person can help the member manage these services
